I think it was defined in the original Mac user interface guidelines. Essentially it's inserting (or deleting) space where it makes sense to.

I was unable to run it under Mac OS X 10.4. I have X11 installed, and that opened when I 2clicked your app. But I never saw any NoteCase window.

Hmm, this seems like the sort of thing you'd put in a subtopic or a column. In my mind, footnotes are more for a final formatted document, and that's not usually what I use an outliner for.
