I forgot about the multimedia part. But I’m afraid I don’t have any knowledge about that.

One more thing I forgot to mention: I’m not sure, but from your description is sounds as if you may want topics and note text to show up without hierarchical indentation in the so-called “Document View”. However, with all the options I mentioned, the hierarchical indentation remains. Microsoft Words is the only option that I know of that offers both an outline view and a non-indented document view.
