My team has been doing a lot of work in this area.

An outline is a major part of our web project management tool (http://www.pyra.com). We store all the items in a relational database, but we also maintain an XML version of the outline for each project so that we can spit it out in HTML form much more efficiently. It greatly speeds up our application. (The other advantage, of course, is it allows us to offer the XML to let others hook into our application.)

I don’t think this is exactly what you were talking about, but it seems closely related. And it works well.
